Dear Parent or Guardian,

As one of the most influential people in your child’s life, you have the opportunity to help him or her understand the importance of preparing for life after high school. We are committed to college and career readiness for all students. In compliance with Section 1111(b)(2)(H) of ESSA, Foyil School would like to provide community stakeholders with an opportunity to provide meaningful input regarding our district’s intent to administer the ACT in lieu of the statewide assessment for all grade 11 students during the Oklahoma State testing window in April 2021. Located on the Foyil School website, foyil.k12.ok.us, there is an ACT Parent Survey listed under “Latest News” where parents may provide feedback regarding the Foyil School District’s intent to choose the ACT assessment. If online services are not available, we have attached a survey for you. We are required to collect a consent form from a parent/guardian for your child to fill out the non-test information that is required when taking the ACT assessment. Please find the enclosed form, sign and return to Foyil School. Together we will prepare your student to be college and career ready. ACT SURVEY
